What is Emsella?
Emsella is a non-invasive medical device developed by BTL Industries,
cleared for the treatment of urinary incontinence and pelvic floor dysfunction in both women and men.
The technology uses High-Intensity Focused Electromagnetic (HIFEM) energy to stimulate deep pelvic floor muscles,
structures that are often difficult to engage through voluntary exercise alone.
The patient remains fully clothed throughout the session, seated in the Emsella chair.
No probes, no undressing, no downtime.
How Does It Work?
The Emsella chair generates a rapidly alternating electromagnetic field with a peak intensity of 2.5 Tesla. This field penetrates the pelvic basin and induces involuntary contractions of the levator ani muscle complex, the primary supportive structure of the pelvic floor.
A single 28-minute session produces approximately 11,000 supramaximal muscle contractions, a level of muscular recruitment that cannot be achieved through voluntary Kegel exercises, even when performed correctly.
Over the course of the treatment protocol, this intensive stimulation leads to:
Muscular hypertrophy
an increase in the size and strength of pelvic floor muscle fibres
Neuromuscular re-education
restoration of the brain-to-muscle signalling pathways that control continence
Improved urethral support
reducing involuntary leakage during physical activity, coughing, or sneezing
Unlike traditional electrostimulation (which requires transvaginal probes and is limited by skin resistance), electromagnetic energy bypasses superficial tissue entirely, reaching deep motor neurons without discomfort.
Clinical Indications
Emsella is indicated for patients presenting with:
- Stress Urinary Incontinence (SUI) - Involuntary leakage triggered by coughing, sneezing, laughing, or physical exertion, typically caused by weakened pelvic floor support following childbirth or hormonal changes.
- Urge Urinary Incontinence (UUI) / Overactive Bladder - A sudden, compelling urge to urinate driven by neurological detrusor overactivity. HIFEM stimulation activates an inhibitory reflex that helps calm bladder activity.
- Post-Prostatectomy Incontinence in Men - Men experiencing persistent urinary incontinence following radical prostatectomy are eligible candidates. Clinical data shows a 60.6% sustained reduction in incontinence severity scores at one-month follow-up, with significant reduction in daily pad use.
- Postpartum Pelvic Floor Rehabilitation - Women who have completed initial postpartum recovery (typically 6 weeks to 6 months following delivery) may benefit from proactive pelvic floor restoration, before symptoms of dysfunction become established.
- Mild to Moderate Pelvic Organ Prolapse (Stage I–II) - In cases where surgical intervention is not indicated or desired, Emsella can provide mechanical support by increasing the overall tone and lift of the pelvic floor musculature.
- Mixed Urinary Incontinence - Patients presenting with both stress and urge components benefit from the dual mechanism of the therapy.
The Treatment Protocol
- Sessions 6

- Clothing - Fully clothed throughout
- Downtime - None, daily activities resume immediately

The intensity is adjusted gradually at the start of each session, titrated to the patient’s individual tolerance.
The procedure is not painful.
Mild muscle fatigue in the pelvic area may be experienced in the hours following a session,
a physiological response consistent with muscular exertion. This resolves without intervention.
Contraindications
Emsella is not suitable for patients with any of the following:
- Pregnancy or planned pregnancy
- Cardiac pacemaker or implanted defibrillator
- Metal implants in the pelvic or lumbosacral region
- Copper intrauterine device (IUD)
- Active or untreated malignancy
- Severe pelvic organ prolapse (Stage III or above)
- Bleeding disorders or active haemorrhage
- Epilepsy
- Metal piercings between the waist and knees (must be removed prior to treatment)
- Recent pelvic surgery (healing must be complete before commencing treatment)
A full medical history review is conducted prior to the first session. Contraindication screening is mandatory and non-negotiable.
Who Should Consider This Treatment?
Emsella may be relevant for patients who:
- Experience urinary leakage during physical activity, coughing, or sneezing
- Have completed postpartum recovery and wish to restore pelvic floor tone
- Are entering or have completed menopause and notice changes in bladder control
- Prefer a non-surgical, non-invasive approach
- Have previously attempted pelvic floor physiotherapy with limited results
- Are men recovering from prostate surgery with persistent incontinence
