What is Colposcopy and why does it matter?
Colposcopy is the diagnostic examination performed when a cervical screening test returns an abnormal result, whether an abnormal smear (cytology) or a positive high-risk HPV test. During the examination, the cervix is examined under magnification to identify pre-cancerous lesions and, where necessary, obtain a targeted biopsy.
The accuracy of this examination is critical. Standard colposcopy relies on the clinician’s visual assessment of the cervical tissue, a process that is inherently subjective.
Nexxmedis uses the Dysis Digital Colposcope, a computer-aided system that replaces subjective visual assessment with objective, real-time cervical mapping.
What Makes Dysis Different?
Conventional colposcopy captures a static image of the cervix at a single moment in time.
The clinician observes the tissue and makes a visual judgment.
Dysis goes further.
The system measures how the cervical tissue behaves over time, tracking the speed, intensity, and duration of the tissue’s response to acetic acid (a standard diagnostic agent applied during any colposcopy).
This process is called Dynamic Spectral Imaging (DSI).
The data collected is processed pixel by pixel by an onboard algorithm, producing a colour-coded map of the cervix,
the DYSISmap, displayed in real time on a 15-inch touchscreen monitor.
The DYSISmap translates tissue behaviour into a visual gradient:
Cyan / Blue / Green
Low-grade or normal tissue response
Yellow / Red / White
Intense, rapid, sustained response, associated with high-grade dysplasia
This map does not replace the clinician’s judgment.
It provides an objective, standardised layer of information to guide biopsy site selection and clinical decision-making.
How the Technology Works
When acetic acid is applied to the cervix, abnormal cells,
which have a higher nuclear density, react differently from healthy tissue.
They turn white faster, more intensely, and for longer.
The Dysis system captures this response continuously for 2 to 3 minutes, measuring three parameters at the pixel level:
Onset speed
how quickly the whitening begins
Maximum intensity
the peak brightness of the reaction
Persistence
how long the whitening effect lasts
High-grade dysplastic tissue (CIN2, CIN3) exhibits a characteristically rapid,
intense, and prolonged response. The DYSISmap makes this pattern visible and documentable
in a standardised format that can be compared across visits.
Clinical Indications
A Dysis colposcopy examination is indicated for patients referred following:
- Abnormal cervical cytology — including ASC-US, LSIL, ASC-H, HSIL, or atypical glandular cells (AGC)
- High-risk HPV positivity — particularly HPV genotypes 16 and 18, even when cytology is negative
- Discrepancy between cytology and visual findings — cases where a high-grade smear result does not correspond to what is visible on standard examination
- Conservative surveillance of confirmed CIN2 — in young, nulliparous women where the clinical decision is to monitor rather than treat, Dysis enables structured longitudinal tracking of lesion behaviour over time
What to Expect
The Dysis examination follows the same steps as a standard colposcopy, with the addition of automated digital mapping.
The examination typically takes 15 to 20 minutes in total.
- Before the appointment - Patients are advised to avoid vaginal intercourse, tampons, lubricants, and douching for 24 to 48 hours prior to the procedure. Scheduling during active menstruation is not recommended, as this may affect optical visualisation of the cervix.
- During the examination - The patient is positioned as for a standard gynaecological examination. The Dysis imaging head is positioned at a standardised distance from the cervix. A baseline digital image is captured and saved to the patient's record.
- Acetic acid is applied via the system's integrated, standardised applicator, ensuring a consistent, homogeneous coverage of the transformation zone. The system then automatically initiates a 2 to 3-minute continuous scan.
- During this window, the clinician examines the cervix as they would in any colposcopy, using digital magnification, green and high-contrast optical filters, and standard vascular assessment criteria. Any areas of clinical concern are marked directly on the touchscreen.
- At the end of the scan, the DYSISmap is generated and overlaid on the live cervical image. Biopsy sites are selected based on both clinical indicators and the colour map. Biopsy locations are recorded digitally on the patient's record.
- After the examination A standardised PDF report, containing high-resolution images, the DYSISmap, and annotated biopsy locations, is generated and stored in the patient's medical file. This report can be compared directly with examinations at future visits.
Longitudinal Tracking: SMARTtrack
For patients who require surveillance over time,for example,
those under conservative management for CIN2, the Dysis system includes SMARTtrack software.
This feature allows the clinician to load previous cervical maps alongside the live current examination,
enabling a direct, side-by-side comparison of tissue behaviour across visits.
Changes in the acetowhitening pattern, whether regression or progression, are visible
in a standardised, reproducible format.
This approach supports informed clinical decision-making about whether to continue surveillance
or proceed to treatment, without relying on memory or subjective comparison.
Who Should Consider This Treatment?
Dysis colposcopy is recommended for women who have received an abnormal Pap smear result, a positive HPV test, or an inconclusive cervical screening.
It is also indicated for those with unexplained pelvic symptoms or a history of cervical dysplasia requiring closer monitoring.
